Redesigning Primary Care

The Patient-Centered Medical Home (PCMH) is a model of primary care delivery that has the potential to improve clinical quality, improve patient experience and reduce health system costs. In a PCMH, patients have enhanced access to a care team that provides proactive, evidence-based, coordinated and whole person care. PCMH practices measure their performance and conduct quality improvement activities to improve it. PCMH practices focus on the needs, preferences, and abilities of patients and families by engaging them in their own care and in the practice redesign process.
