Arkansas Payment Improvement Initiative's PCMH Program

In support of the Arkansas Payment Improvement Initiative (APII), the state has selected Qualis Health to provide patient-centered medical home (PCMH) consulting services to primary care practices throughout Arkansas.

Enroll in the Qualis Health PCMH Practice Support Program

Through the APII, practices across the state are now eligible to receive monthly care coordination payments and support from the state's only pre-qualified consultant, Qualis Health, to help adopt the PCMH Model of Care and achieve Medicaid's PCMH metrics.

For the APII project, our consulting staff will assess your practice, then tailor our approach and resources to best fit your needs. Our flexible approach will help you:

• Take full advantage of new payment models
• Increase provider and staff satisfaction
• Help improve the quality of care provided to all patients

By joining the Program, you will receive:

• 30 hours of one-to-one coaching from a PCMH expert
• PCMH implementation needs assessment
• PCMH implementation plan tailored to your practice’s needs
• Live online learning events, plus access to an archive of recorded sessions
• Periodic in-person learning sessions for expert training and peer-to-peer networking
• Library of PCMH implementation tools and resources
