Safety Net Medical Home Initiative (SNMHI)


In May 2008, The Commonwealth Fund, Qualis Health and the MacColl Center for Health Care Innovation at the Group Health Research Institute launched the Safety Net Medical Home Initiative, a landmark demonstration of the PCMH Model of Care. The goal of the Initiative was to develop a model of practice transformation and help 65 primary care safety net sites in five states improve quality, efficiency and patient experience.
