
L.A. Care Health Plan (Local Initiative Health Authority of Los Angeles County) is a public entity and community-accountable health plan serving more than one million residents of Los Angeles County.
Qualis Health is providing targeted technical assistance to help transform 11 L.A. Care Health Plan network practices into high-functioning patient-centered medical homes. Qualis Health helped design and initiate the program by recruiting sites, analyzing proposals, and helping select participants. We also assessed practices' transformation readiness and prepared a gap analysis and individual technical assistance plan for each. We have continued our support with customized technical assistance (e.g., workshops and coaching on all domains of the PCMH Model, including empanelment). We are also helping participating practices attain NCQA PCMH™ Recognition.

Evidence-Based Guidelines: Percentage of practices that have identified and make use of evidence-based guidelines for at least three chronic or recurring conditions
High-Risk: Percentage of practices with a defined criteria and a process for proactively identifying high-risk patients within the practice population
