Advanced Primary Care Practice Demonstration Project (APCP)

The American Institutes of Research selected Qualis Health as its technical assistance partner for the CMS FQHC Advanced Primary Care Practice (APCP) Demonstration Project.

The goal of the APCP Demonstration is to improve care, promote health and reduce the cost of care provided to Medicare beneficiaries served by Federally Qualified Health Centers (FQHCs). Participating FQHCs are expected to achieve Level 3 2011 NCQA PCMH™ Recognition by October 2014.

Qualis Health is working directly with FQHCs and state Primary Care Association practice coaches to help participants document policies and procedures and prepare application materials to demonstrate their implementation of the PCMH Model of Care.
