Webinars & Other Tools for Becoming a Medical Home The Safety Net Medical Home Initiative has hosted many informational webinars, and created a variety of tools, to help participating clinics manage change and assess their progress toward becoming medical homes. These "change concept" resources are also available, free of charge, to the general public.
PCMH Assessment Tool The Patient-Centered Medical Home Assessment (PCMH-A) is a self-assessment tool created by the SNMHI to allow practices to gauge their progress in implementing each of the change concepts. The PCMH-A is an interactive PDF that can be downloaded, completed, saved and shared.
