The following healthcare policy resources were developed by the Safety Net Medical Home Initiative.
Policy Brief 1 - Paying for the Medical Home: Payment Models to Support Patient-Centered Medical Home Transformation in the Safety Net (October 2010)
In order for patient-centered medical home practice transformations to be sustainable, there must be payment reform to incentivize high-value, first-contact, primary care, and support medical home costs that are traditionally not reimbursed. This brief provides an introduction to payment reform opportunities to support and sustain the medical home.
Policy Brief 3 - Paying for the Medical Home Part 2: Considering Social, Behavioral, and Environmental Factors in Payment Models (August 2011)
Primary care payment models that do not account for patient/family social, behavioral or environmental factors (which require additional provider/staff time and resources to address), in addition to health factors, may put both providers and patients at a disadvantage. This policy brief presents ideas about how payment models can be improved to better support patient-centered medical home transformation in the safety net.
