Released by the Patient Centered Primary Care Collaborative (PCPCC), the Purchaser Guide is a handbook for understanding the medical home model and taking action to advance its implementation.

This site houses NCQA’s Physician Practice Connections: Patient-Centered Medical Home™ resources, including the PPC-PCMH joint principles, standards and guidelines and assessment tools.
- TransforMED
TransforMED™ provides a free medical home assessment for physicians and group practices. The survey asks for information in eight different domains: access to care, continuity of care, information systems, patient-centered care, point of care services, practice management, quality and safety, and team-based care. It then computes module-specific and summary scores, and provides links to resources and information.
