Change Concepts for Practice Transformation

Qualis Health, in partnership with the MacColl Center for Health Care Innovation and with the support of The Commonwealth Fund, developed a framework for medical home transformation. This framework, called the “Change Concepts for Practice Transformation,” has been extensively tested by the 65 practices participating in the Safety Net Medical Home Initiative and used by other collaboratives and practices nationwide. The framework is supported by a comprehensive library of training materials that provide detailed descriptions and real examples of transformation strategies. These resources are free and publicly available.

Change concepts are general ideas used to stimulate specific, actionable steps that lead to improvement. Our medical home framework includes eight concepts:
