Southside Coalition of Community Health Centers PCMH Initiative

In April 2011, Qualis Health kicked off a 24-month PCMH transformation project to support the Southside Coalition of Community Health Centers in Los Angeles, California, improve efficiency, clinical quality, service capacity and patient health outcomes. Supported by The California Endowment, this project uses the NCQA PCMH™ Recognition standards as a framework for sustainable operational change.
