The REACH Healthcare Foundation contracted with Qualis Health to launch a bi-state medical home initiative, centered in the Kansas City metropolitan area. Using the NCQA PCMH™ Recognition standards as a framework, Qualis Health is providing participating safety net practices with on-site practice consultation and workshops on PCMH topics, including empanelment; enhanced access; team-based care; care management and care coordination; performance improvement and change management; enhanced skills for medical assistants to support team-based care; and patient-centeredness.
