
The REACH Healthcare Foundation contracted with Qualis Health to launch a bi-state medical home initiative, centered in the Kansas City metropolitan area. Using the NCQA PCMH™ Recognition standards as a framework, Qualis Health is providing participating safety net practices with on-site practice consultation and workshops on PCMH topics, including empanelment; enhanced access; team-based care; care management and care coordination; performance improvement and change management; enhanced skills for medical assistants to support team-based care; and patient-centeredness. Through targeted coaching, Qualis Health has helped practices enrolled in the REACH Medical Home Initiative make substantial improvements in patient care in just under two years.

Access measure: Percentage of practices that have capacity to see patients the same day they call.
Population Management measure: Percentage of practices using a registry application for population management for at least one chronic condition.
Care Coordination measure: Percentage of practices that review information from ED and hospital to determine patient's follow up needs.
Planned Care measure: Percentage of practices that conduct pre-visit planning using clinician reminders and other resources/tools.
Performance Reporting measure: Percentage of practices that regularly report on patient experience, access measures, clinical process measures and clinical outcomes measures.
