Qualis Health has also provided support to State Primary Care Associations that are helping community health centers in their regions become patient-centered medical homes. Typical services include:

• Introductory workshops on medical home concepts
• Assessment, gap analysis and technical assistance planning
• Webinars and telephonic coaching for continued progress
