
Qualis Health is providing targeted technical assistance to help transform 11 L.A. Care Health Plan network practices into high-functioning patient-centered medical homes. L.A. Care Health Plan (Local Initiative Health Authority of Los Angeles County) is a public entity and community-accountable health plan serving more than one million residents of Los Angeles County. Qualis Health helped design and initiate the program, including recruiting participating sites, analyzing proposals, helping select practices, assessing practices' transformation readiness and preparing a gap analysis and individual technical assistance plan for each practice. We have continued our support with customized technical assistance (e.g., workshops and coaching on all domains of the PCMH Model, including empanelment). We are also helping participating practices attain NCQA PCMH™ Recognition.

Access measure: Percentage of practices that have capacity to see patients the same day they call.
Population Management measure: Percentage of practices using a registry application for population management for at least one chronic condition.
Team-Based Care measure: Percentage of practices that use non-physician staff to execute standing orders.
Care Coordination measure: Percentage of practices that have closed-loop referral tracking in place.
