
The American Institutes of Research selected Qualis Health as one of three technical assistance partners for the CMS Federally Qualified Healthcare Center Advanced Primary Care Practice (APCP) Demonstration Project.
The project includes evaluation of the advanced primary care practice model, referred to as the patient-centered medical home (PCMH) model. The model was created to improve care, promote health and reduce the cost of care provided to Medicare beneficiaries served by Federally Qualified Health Centers (FQHCs). Participating FQHC sites are expected to achieve Level 3 2011 NCQA PCMH™ Recognition and complete self-assessments twice a year. In partnership with the National Association for Community Health Centers and the MacColl Center for Healthcare Innovation at the Group Health Research Institute, Qualis Health has developed a support program to help the 500 participating FQHCs meet these goals.
Qualis Health and its partners are also guiding the regional capacity for PCMH transformation by providing "train-the-trainer" support for State Primary Care Association coaches and facilitators. Learn more about our work with State Primary Care Associations.
