Learning Collaboratives

Qualis Health has extensive experience in designing and facilitating learning collaboratives. We have conducted Breakthrough Learning Series Collaboratives and other types of collaboratives on a variety of topics, as well as established and facilitated “learning communities” and “communities of practice” specific to PCMH transformation.

We lead the Safety Net Medical Home Initiative, a demonstration project sponsored by The Commonwealth Fund and conducted in partnership with the MacColl Center for Healthcare Innovation that supports PCMH transformation in the safety net. This project uses a modified learning collaborative approach in which we provide direct technical assistance to participating practices; train community-based Medical Home Facilitators to enhance regional capacity; and facilitate a multi-modal learning network that provides in-person and e-learning opportunities, a resource sharing platform and facilitated practice connections (e.g., field trips). This approach has proven successful in enhancing site-to-site knowledge transfer and tool sharing. Our multi-modal approach also minimizes travel and out-of-practice time for participating sites, while still providing opportunities for individual, small and large group interaction—both face-to-face and telephonically.
