Qualis Health provides patient-centered medical home (PCMH) technical assistance services to healthcare providers and practices. Completion of a self-assessment tool is one of the initial steps a practice takes in the transformation process. We use different self-assessment tools depending on the needs and objectives of clients.
Qualis Health uses the Patient-Centered Medical Home Assessment (PCMH-A) to help practices understand and measure their medical home transformation. The PCMH-A was developed by the MacColl Center for Health Care Innovation at the Group Health Research Institute and Qualis Health for the Safety Net Medical Home Initiative. The PCMH-A includes eight medical home domains (e.g., enhanced access) and auto-scores on 32 medical home elements (e.g., 24/7 access to care team). For practices interested in trending their progress over time, or for groups of practices interested in comparing their results and identifying collaborative learning opportunities, we offer analysis and comparative reporting.
For practices primarily interested in NCQA PCMH™ Recognition, we offer customized readiness assessments and ongoing progress monitoring tools. After a practice completes a self-assessment, we create a customized technical assistance plan to guide further progress. We use an action plan template to document steps to improvement towards full adoption of the PCMH Model of Care.
Qualis Health uses several tools to re-assess and track progress toward PCMH transformation. These include monthly, quarterly or semi-annual assessment reports, status reports and leadership summaries. We can also produce reports that detail the current status of activities, significant accomplishments for the current period, planned activities for the next reporting period and an issues log.
