About Us

A recognized leader in patient-centered medical home (PCMH) transformation, Qualis Health has developed proven approaches and tools to assist practices. Qualis Health has supported more than 1,000 practices across the country in redesigning their clinical and administrative processes to improve clinical quality, patient experience, provider and staff satisfaction and operational efficiency. We also help practices achieve NCQA PCMH™ Recognition. We lead multi-state demonstration projects, support regional collaboratives and provide customized practice-level assistance for local networks. Our Approach

In 2008, Qualis Health co-developed a PCMH transformation framework, The Change Concepts for Practice Transformation. This framework has been intensively tested in 65 practices and used by dozens of other practices and collaboratives nationwide. It is supported by a comprehensive library of training materials that provide detailed descriptions and real examples of transformation strategies. These resources are free and publicly available. Change Concepts for Practice Transformation

Change concepts are general ideas used to stimulate specific, actionable steps that lead to improvement. Our medical home framework includes eight concepts:

Transformation Support

We work collaboratively with our clients to assess their needs and develop a customized PCMH support package. Typical services include:

• Assessment and Planning
  We conduct assessments to determine the readiness of practices to implement the PCMH Model using a tool appropriate for the interests and needs of the client (e.g., PCMH-A, NCQA PCMH™ Recognition Readiness Assessment). This includes an analysis of existing gaps and preparation of technical assistance plans to address those gaps. Learn more about our assessment and planning services.
• Consultation Services
  We deliver professional consultation and technical assistance services to support and empower clinical practices through the stages of redesign that evolve PCMH transformation into a sustainable model of practice. In addition to providing technical assistance on a full spectrum of PCMH topics, our consultants help clients identify and solve problems in the interest of improving healthcare delivery and administrative practices, systems of care, cost and quality. • Practice Coaching
  We provide individual and group coaching on PCMH concepts with a focus on implementation assistance. Coaching may include telephonic or on-site consultation, webinars, workshops and learning collaborative-style events. To promote local and regional capacity and foster sustainability, we also provide train-the-trainer services that focus on skills-building for future practice coaches. • Learning Collaborative Management
  For learning collaborative projects, we offer full-scope administration services. We can develop selection criteria for participants, assist with the recruitment and selection of qualified practices, support learning communities, plan and conduct in-person and distance learning events, and monitor and evaluate progress. • Recognition Support
  We provide comprehensive support for practices pursuing NCQA PCMH™ Recognition including administration of a readiness assessment, recommendations for improvement, assistance developing policies and protocols, final review and submission support. Focus on the Safety Net

Qualis Health has extensive experience in providing transformation support for safety net practices: Federally Qualified Healthcare Centers (FQHCs), homeless health centers, migrant and seasonal farmworker health centers, free clinics, critical access hospitals, residency programs and private practices that serve the uninsured and Medicaid beneficiaries.

Through the Safety Net Medical Home Initiative, sponsored by The Commonwealth Fund and conducted in partnership with the MacColl Center for Health Care Innovation at the Group Health Research Institute, we developed a library of transformation resources for safety net practices.
